Public Records Request
Public records requests are for information that currently exists and have been created in the normal course of business. The Authority follows the State laws that regulate the retention and destruction of records.
Non-Medical Records Request
Before you submit your request, please confirm the incident took place within the Arizona Fire & Medical Authority's jurisdiction. The Authority proudly serves Buckeye Valley, Coldwater Ranch, Corte Bella, Cross River, Dos Rios, Guadalupe, Pinnacle Peak County Island, Rancho Cabrillo, Rancho Silverado, Rio Sierra, Sun City West, Sun Lakes, Sundero, Tonopah, and Wittmann.
If the incident did not occur within the above areas, please contact the fire/emergency medical services agency that responded to the incident.
Records requests can be submitted online and emailed to info@afma.az.gov.

Medical Records Request
The Authority complies with the Health Insurance Portability and Accountability Act (HIPAA), including the breach notification rules of the Health Information Technology for Economic and Clinical Health (HITECH Act). The Authority has in place policies and procedures for using and disclosing patient information, safeguarding patient information, and for providing patient access. The Authority may use or disclose your protected health information (PHI) without your written authorization in limited situations. Please refer to the Authority's Notice of Privacy Practices for details.

Town of Guadalupe Public Records Requests
If you are seeking fire incident records for a time period before October 10, 2025, please contact the Town Clerk’s Office. Information and request forms are available on the Town’s website.
For fire incident records dated after October 10, 2025, please contact the Arizona Fire & Medical Authority through the records request page.
